Root cause analysis is the use of certain methods to solve problems. The practice seeks to find the main cause of the problem instead of addressing the symptoms. By addressing the main cause of a problem and providing a solution, it becomes very hard for the problem to recur. Organizations use root cause analysis as a tool for continuous development. This tool enables organizations to detect problems and solve them. They therefore perform the analysis after a problematic event occurs. However, by gaining expertise in the practice, organizations can use root cause analysis to prevent the occurrence of future problems. A root cause analysis template will act as an important guide in ensuring that the practice of performing the analysis is simple.

This template will guide the people performing root cause analysis on the specific method to use. There are five different methods to use, which include safety-based, process-based, production based, systems-based and failure based root cause analysis. However, there are general principals that guide organizations in performing root cause analysis. These principles are that organizations should perform root cause analysis systematically. They should follow the steps necessary to complete an investigation that will bring out documented conclusions. The hardest part in performing this type of analysis is getting to the main cause of the problem, but by using a root cause analysis template, the organizations performing the analysis will have the necessary informative procedures that they will follow to reach to the problem’s cause. This template will guide them in establishing a timeline and processes that will lead them in understanding the relationship between the factors that contribute to a problem and the reasons why the problem occurred.

The benefit of root cause analysis is that it changes old cultures that react to problems transforming them into new cultures that seek solutions to problems before they get worse. It also reduces the chances or problem reoccurrence in the areas where organizations continue performing the analysis. The general method of performing root cause analysis involves defining the problem, gathering evidence, asking questions that seek to identify the real cause of a certain problem and seeking corrective actions necessary to prevent problem reoccurrence. The other steps to take are implementing the recommendations gathered from the solutions reached and observing them.

A root cause analysis template will assist an organization to achieve an accurate root cause analysis by aiding it in providing information on how to collect statistical analysis such as linear regressions, psychological path analyses and correlations. This will ensure that the analysis contains a sampling error that is less than one percent. Organizations can also use root cause analysis to assess customer perceptions and identify the perceptions that influence customers to behave in a certain manner. They can also use root cause analysis to identify employee attitudes, beliefs and opinions that influence employee behavior. They will know those behaviors that affect customer return and look for ways to ensure that these behaviors do not affect their sales and other business transactions.
